Our primary focus is to provide access to health assessments and access to medical care for all W&J学院 students and promote your wellbeing. 学生健康 Services Center provides short-term, solution-focused, outpatient care. If you need care beyond the scope of the services our Center typically provides, we will work with you to find medical care and treatment through local community providers qualified to meet your needs and develop treatment plans in collaboration with those providers.
College Medical Requirements
To ensure the health and wellness of the whole community, you must complete a 大学医疗表格 and provide a Physical Evaluation Form from your family physician verifying a recent physical exam and an up-to-date immunization history. Your immunization documentation must include the dates of these three required immunizations:
- MMR- 2剂 (麻疹,腮腺炎 & 风疹)
With the recent increase in reported Measles cases, your MMR immunization must be current. - 乙型肝炎系列(3)
- Meningococcal Meningitis
A booster dose is required if you received your initial dose before to age 16. If you were immunized after age 16, only a single dose required.
IF YOU HAVE NOT MET THIS CRITERIA, YOU MUST SIGN THE DECLINATION ON THE ACKNOWLEDGMENT OF MENINGITIS VACCINE INFORMATION FORM.
